Student trip consent form checklist

Not a form to print as-is. A checklist of the fields and separate consents your own form needs, with the points your lawyer should confirm under India's DPDP Act.

What each column is for

Section and field
Trip, student, guardian, emergency, medical, consent, rules, sign-off. Forms that bundle everything into one signature miss the separate consents.
Required
Allergies and medication are required even when the answer is "none". A blank is ambiguous.
Who fills it
Organiser fills trip facts; guardian fills student, medical and consent sections. Students over a certain age may co-sign the code of conduct.
Notes
Where a field exists because of a legal requirement, the note says which, so counsel can check it.

How to use it

  1. Download the CSV and compare it with your current consent form.

  2. Add the missing fields, especially the second guardian number and an emergency contact who is not a guardian.

  3. Split consents: participation, photography, personal data, location sharing. One signature per consent.

  4. Send the draft to counsel with the DPDP notes column.

  5. Collect the signed forms before invites go out and keep them with the roster.

Mistakes this template prevents

  • One signature covering participation, photos and data collection together.
  • No alternate contact when both guardians are unreachable.
  • Collecting location consent without a clear statement of when tracking is on and when it stops.
  • Keeping signed forms in a WhatsApp album.

Questions about this template

What should a school trip consent form include?

Trip details with every overnight stay, student identity and date of birth, two guardian phone numbers, an alternate emergency contact, allergies, medication and conditions, consent to emergency treatment, and separate consents for listed activities, photography, personal data collection and any location sharing, followed by signatures and dates.

Does the DPDP Act affect student trip forms?

Yes. India's Digital Personal Data Protection Act 2023 treats anyone under 18 as a child and requires verifiable consent from a parent or lawful guardian before processing a child's personal data. Section 9 also restricts tracking and behavioural monitoring of children, subject to exemptions in the Rules. Have counsel confirm how your form and any location feature fit.
